For a national visa D to Germany, you usually need a document that confirms medical coverage from the first day of entry. If German public or private insurance has not started yet, this gap is covered by an Incoming-Versicherung policy: temporary entry insurance for foreigners.
Incoming does not replace German Krankenversicherung for the period of living and working in Germany. Its role is narrower: to cover the first days or weeks after entry while a person registers, starts work, activates student or work insurance, and gets normal access to the German healthcare system.
When Incoming-Versicherung is needed
Most often, entry insurance is arranged in three situations:
- an employee is traveling to Germany on a work visa, but public insurance will only start on the first working day;
- a family is entering together with the main applicant, while family insurance will only start after address registration and the family members are added to the policy;
- a student, prospective student, or another applicant must show the consulate medical coverage before full German insurance begins.
If a German Krankenkasse has already issued proof of insurance for the visa, Incoming may still be needed for the period between actual entry and the start date of the main insurance. This is especially important when the family arrives earlier, while work or studies start later.
How Incoming differs from travel insurance
Incoming-Versicherung is similar to travel insurance, but it is chosen specifically for entry and temporary stay in Germany. In the contract, you should look not at the product name, but at the terms: country of coverage, duration, coverage for illness and injuries, how bills are paid, exclusions, deductible, and whether the policy can be used for a national visa.
Ordinary insurance from the country of departure may not be suitable if it is designed only for a short trip and does not cover a long-term move. In that case, a person risks getting a visa “just for show,” but when actually visiting a doctor, paying out of pocket and then arguing with the insurer over reimbursement.
Before buying, check:
- whether the policy is valid specifically in Germany and for the entire required period;
- whether this format is accepted by the specific consulate or visa center;
- whether outpatient and inpatient treatment are covered;
- how to report an insured event: through a voucher, direct billing, reimbursement after payment, or a mixed scheme;
- whether chronic illnesses, pre-existing diagnoses, pregnancy, dental care, psychotherapy, or sports are excluded;
- whether you can move the start date or terminate the contract early after German insurance is activated.
How a claim works
The terms depend on the specific company. In some products, the doctor or clinic contacts the insurer directly; in others, the insured person first pays the bill, collects the documents, and files a reimbursement request. That is why, before visiting a doctor, it is useful to open the contract and find the sections on claims, reimbursement, emergency assistance, and billing.
Incoming is usually designed for new illnesses and injuries that arise while the policy is in force. Pre-existing conditions, planned treatment, expensive operations, chronic conditions, and treatment that could have been foreseen before entry are often excluded or covered only to a limited extent. With a serious diagnosis, you should not assume that temporary entry insurance can replace full German Krankenversicherung.
How long to arrange the policy for
It is better to calculate the duration based on the real entry scenario, rather than automatically buying six months for every family member.

For example, if the contract starts on October 1 and the family arrives on September 15, the logic is this: the main German insurance must be confirmed for the job, while Incoming covers the period from September 15 to September 30. For family members, the period may be longer if address registration and inclusion in family insurance take time.
Incoming and German public health insurance
Health insurance is mandatory in Germany. For employees, the main split is usually this: with standard employment, a person generally enters the gesetzliche Krankenversicherung (GKV) system if their income does not exceed the current Versicherungspflichtgrenze. If income is above that threshold, a choice between voluntary participation in GKV and private Krankenversicherung (PKV) is possible, but the decision on private insurance is better made after consultation, because returning to GKV can be difficult.
Public insurance usually starts on the first working day. Doctors and pharmacies are then shown the electronic eGK card, and many services are paid directly through the Krankenkasse. For non-working spouses and children, Familienversicherung may be available without a separate contribution if the family insurance conditions are met.
Incoming in this scheme is needed only as a bridge. It does not provide full GKV participant status, does not replace health and pension insurance contributions, and does not solve the employer’s issues with Sozialversicherungsnummer, Steuer-ID, and other registration data.
What to ask the insurer before buying
Before paying for the policy, ask the insurer or broker specific questions and keep the answers in writing:
- whether the policy is suitable for a national visa D in your category;
- what start date to enter if the exact day of entry is still unknown;
- whether it is possible to shorten the term and get part of the premium back after GKV or PKV starts;
- what documents are needed for reimbursement;
- whether there are limits on dental care, repatriation, transport, pregnancy, and chronic illnesses;
- whether the insurance is valid in other EU countries or only in Germany;
- how to urgently contact assistance in case of hospitalization.
Care Concept AG, Mawista, DR-WALTER, and other German providers are common in these scenarios, but you need to compare not the brand, but the specific tariff and Versicherungsbedingungen. Exact prices, age limits, and coverage caps change, so it is safer to check them in the insurer’s current offer before buying.
Common mistakes
Buying the cheapest policy without checking the terms
For the visa, it is not just the PDF with a nice title that matters. If the contract does not cover long-term entry, treatment in Germany, or the required period, the savings may end with a doctor’s bill.
Taking Incoming for six months “just in case”
Sometimes this is really required, but often it is enough to cover the short gap before GKV, PKV, or student insurance starts. Extra months increase costs, especially for a family.
Assuming family insurance is active from the moment of entry
Familienversicherung usually requires formal processing and confirmation that the conditions are met. Before address registration and document processing, family members may have an uncovered period.
Not checking the start date
If the policy starts later than the actual date of entry, the first days remain uncovered. If the entry is postponed, you need to understand whether the insurer automatically shifts the coverage period or requires a separate request.
Short checklist before the visa application
- Check the checklist for your specific consulate and visa category.
- Obtain confirmation of the main German insurance if it is already possible.
- Calculate the uncovered days between entry and the start of the main insurance.
- Select Incoming for this period for every person who needs it.
- Make sure the policy correctly states the name, date of birth, country of coverage, and dates.
- Keep the contract, insurance certificate, assistance contacts, and claims filing rules.
- After GKV or PKV begins, clarify whether the temporary policy can be terminated early.
FAQ
Is it possible to enter with only proof of GKV?
Sometimes yes, if the confirmation meets the consulate’s requirements and covers the period from the first day of stay. If there is a gap between entry and the start of GKV, that gap must be covered by separate insurance.
Does a child need a separate policy?
Until the child is added to Familienversicherung, separate temporary coverage may be required. The exact period depends on the date of entry, registration, and document processing by the Krankenkasse.
Does Incoming cover chronic illnesses?
Often no, or only in limited emergency situations. This is one of the main points that must be read in the Versicherungsbedingungen.
Do you need to keep Incoming after starting work?
