MPU in Germany: When It Is Ordered and How the Medical-Psychological Assessment Works
MPU, or Medizinisch-Psychologische Untersuchung, is a medical-psychological assessment of fitness to drive in Germany. It is ordered by the Führerscheinstelle if the authority has doubts that a person can safely operate a vehicle. In everyday speech, MPU is often called Idiotentest, but officially it is an expert assessment of Fahreignung.
An MPU result does not automatically restore your license. The center issues an expert report, and the final decision is made by the Führerscheinstelle. That is why the driver’s main task is not to guess the “right answers,” but to convincingly show that the reason for the violation has been understood, the risk of repetition has been reduced, and the behavioral changes are stable.
When MPU can be ordered
MPU is not ordered for every violation. It is usually used in cases where the authority sees a risk to road safety.
Common reasons:
- alcohol at the wheel, especially with a high blood alcohol level or repeated violations;
- drugs and other psychoactive substances;
- reaching 8 penalty points in the Flensburg register;
- systematic or aggressive traffic violations;
- medical or psychological reasons that may call the ability to drive into question;
- individual cases during obtaining or restoring a driver’s license in Germany.
With alcohol, not only the alcohol level matters, but also the circumstances: repeat incidents, the driver’s behavior, an accident, and combination with other violations. With drugs, the authority separately evaluates the type of substance, evidence of use, and the link to driving.
Who orders MPU and where to take it
MPU is ordered by the Führerscheinstelle. The authority formulates the questions for the experts: for example, whether it can be expected that the driver will again get behind the wheel under the influence of alcohol or drugs.
The assessment itself is carried out by recognized Begutachtungsstellen für Fahreignung. Such centers operate under different providers, for example TÜV, DEKRA, AVUS, and other organizations. BASt publishes the list of recognized locations.
The process usually works like this:
- The Führerscheinstelle informs you that MPU is required to make a decision on your driver’s license.
- The driver chooses a recognized center and consents to the transfer of the driving file.
- The Führerscheinstelle sends the documents to the chosen center.
- The center schedules an appointment after payment for the assessment.
- After MPU, the driver receives the expert report and decides whether to submit it to the Führerscheinstelle.
If the report is negative, it is usually not worth sending it to the authority automatically without consulting a specialist, because it can confirm the doubts about fitness to drive.
What MPU consists of
MPU usually includes three parts:
- a medical examination;
- a test of reaction, attention, and performance;
- an interview with a Verkehrspsychologe, meaning a traffic psychologist.
The assessment often takes several hours. The exact program depends on the reason for the MPU: alcohol, drugs, points, medical restrictions, or repeated violations.
Medical part
The doctor reviews the medical history and asks questions about health, medication, and substance use. If the reason is alcohol or drugs, laboratory tests may be required: blood, urine, hair, or Abstinenznachweise that have already been collected.
It is important to report prescribed medication in advance and bring documents from your doctor if a drug may affect the test results or your condition during the assessment.
Reaction and attention test
Computer-based tasks check reaction speed, concentration, visual perception, and the ability to respond to several signals at once. This is not a traffic-rules exam and not a simulator that expects an “ideal” result. Experts assess whether your current functions are sufficient for safe participation in road traffic.
If the standard test is not passed, the center may offer an additional assessment. The format depends on the specific case and the center’s rules.
Psychological interview
The conversation with the psychologist is the key part of MPU. The expert checks whether the driver understood the reasons for the violation and changed behavior so that the risk of repetition is now low.
For alcohol-related cases, the questions may cover:
- the role of alcohol in the past;
- the amount and frequency of consumption;
- the day of the violation and the decision to drive;
- the difference between controlled use, misuse, and dependence;
- the specific plan for preventing a repeat incident.
For drugs and cannabis, the discussion is usually stricter: you need to show a clear separation between use and driving, and in some cases documented abstinence. After the partial legalization of cannabis, driving under the influence of THC still remains a risky legal and medical issue; stricter rules apply for novice drivers and for mixed use of alcohol and cannabis.
How to prepare
Preparation is not about memorizing answers. The psychologist expects a consistent story: what happened, why it became possible, what conclusions were drawn, and what changes have already become established.
Useful steps:
- request the exact reason for the MPU and the questions for the assessment from the Führerscheinstelle;
- get Einsicht into the driving file if possible;
- attend an Erstberatung with a qualified Verkehrspsychologe;
- start an Abstinenzprogramm early if it is required;
- collect medical documents, certificates, and proof of therapy or consultations;
- review the day of the violation and your earlier behavior honestly;
- avoid courses that promise a guaranteed result.
Template answers from the internet are less useful. If your story sounds memorized and does not match the documents, that reduces credibility.
Evidence of abstinence
Abstinenznachweise are not always required. Whether they are needed depends on the reason for the MPU, the history of use, and the conclusions of specialists. In cases of alcohol dependence, alcohol misuse, drugs, or cannabis dependence, passing the MPU without a proven period of abstinence may be difficult or impossible.
The following are commonly used as proof:
- urine testing within a program with unannounced call-ins;
- hair analysis, if it is suitable for the specific substance and time period;
- medical reports and therapy documents.
Abstinence periods cannot be reliably derived from a general article: they depend on the case, the substance, the diagnosis, and the requirements of the assessment center. That is why it is better to coordinate the program in advance with a qualified consultant or recognized center so that the tests will be accepted for the MPU.
MPU without German
Foreign nationals can take the MPU with an interpreter, but the interpreter usually has to be chosen or approved by the center itself. You cannot simply bring an acquaintance. The driver pays for the interpreter’s services.
Before booking, you need to clarify:
- whether the center works with the required language;
- how much the interpretation costs;
- whether only the psychological interview is interpreted or all parts of the assessment;
- which documents need to be submitted in advance.
Even with an interpreter, it helps to learn the basic German terms: Führerscheinstelle, Fahreignung, Abstinenznachweis, Sperrfrist, Gutachten, Verkehrspsychologe.
How much MPU costs
The costs depend on the reason for the assessment, the center, the preparation, and whether laboratory tests are needed. In 2026, it is better to think not in terms of one fixed price, but several cost blocks:
| Expense | What it includes |
|---|---|
| The MPU itself | Payment for the assessment at the chosen center |
| Preparation | Erstberatung, individual consultations, group sessions |
| Abstinenznachweise | Urine or hair tests, if required |
| Interpreter | If the MPU is not taken in German |
| Führerscheinstelle | Applications and administrative fees related to the driver’s license |
According to ADAC, the final amount often consists of several hundred euros for the assessment plus additional costs for testing and preparation. For alcohol- or drug-related cases, the total can rise noticeably because of the Abstinenzprogramm and individual preparation.
Before paying, ask the center for a written estimate and clarify what happens if the date has to be postponed.
MPU result
After the assessment, the center prepares a Gutachten. In most cases, the driver can specify who should receive the result. Often it is more sensible to receive the report first yourself and then decide whether to submit it to the Führerscheinstelle.
In practice, there are three scenarios:
- a positive report: it is submitted to the Führerscheinstelle for a decision on restoring or issuing the license;
- a negative report: you need to analyze the reasons for the refusal and prepare again;
- a report with a Nachschulung recommendation: in some cases, completing the recommended course is enough if the Führerscheinstelle accepts it.
Retaking the MPU is possible, but each new assessment must be paid for again. If the result is negative, it is better to first eliminate the reasons for the refusal instead of immediately booking another attempt.
Common mistakes
- Booking the MPU before completing the necessary tests and preparation.
- Assuming that cannabis legalization automatically removes the risk to your driver’s license.
- Trying to memorize universal answers instead of honestly analyzing your behavior.
- Not checking whether the center is recognized as a Begutachtungsstelle für Fahreignung.
- Sending a negative report to the Führerscheinstelle without consultation.
- Starting an Abstinenzprogramm too late or in an unsuitable laboratory.
- Ignoring documents about medication, diagnoses, or therapy.
FAQ
Can you pass MPU on the first attempt?
Yes, but the chances depend heavily on the reason, the documents, and the preparation. Simply paying for the assessment does not increase the likelihood of a positive result.
Do you have to take MPU after 8 points in Flensburg?
After reaching 8 points, the driver’s license is revoked. To restore it, the Führerscheinstelle may require proof of fitness to drive, including MPU. It is best to clarify the exact procedure with your local authority.
Cannabis is legal, so MPU is no longer a risk?
No. Partial legalization does not mean permission to drive under the influence of THC. In case of violations, repeated incidents, mixed use with alcohol, or doubts about the ability to separate use from driving, MPU remains possible.
Where can you find official MPU centers?
Check the list of recognized Begutachtungsstellen für Fahreignung from BASt and confirm the center’s current status before booking.
Do you need a lawyer?
A lawyer does not replace MPU preparation, but can be useful if the dispute concerns the legality of the requirement, deadlines, documents, or the actions of the Führerscheinstelle. For the medical and psychological part, you need a specialized Verkehrspsychologe or a recognized advisory service.
